Wrong crafting recipes in guide

The guide book for BM has the crafting recipe for Rune of Self Sacrifice in the sections on Rune of Capacity and Displacement Rune.

Still true in 2.2.12-97. Also, the Ritual Stone entry shows only the recipe for converting colored ritual stones back to blank ones.

And while I'm at it, AFAICT the Alchemy Table is completely undocumented -- that's all the brewing and ore multiplication!
