Crash on Player Death

Description
Occasionally the death of a player will cause the server to crash, it don't always happen, but has happened several times. Both times it happened the death was caused by a mob, however I'm not sure if this is a requirement to cause the crash.
Steps to reproduce
- Step one: player death (may have to be by a mob?)
