I'm playing on the most recent 1.18 version.

When I first created the mining hammer, it worked fine and mined in the expected aoe. However, after some time, the aoe simply stopped working. It then resumed working a couple hours later and hasn't had a problem since.

Toggling the mode of the hammer did not help. Neither did using a different hammer or reloading the game.
