Grave deleting when dying on top of grave

Single Player or Server: Both

Describe problem (what you were doing; what happened; what should have happened):

If you die while on a grave, then no tomb will appear and your items drop. Sometimes it overwrites the older grave so only the new grave exists thus deleting all the items you had on you before the first death.
Reproduced several times on a server and in SP with Enigmatica 6 and in SP with only forge and Simple Tomb
