Only for Java Edition.
There is 3 options to get Connected Glass with Clean and Clear Glass :
A. You can use OptiFine for connected glass.
B. You can use Fabric + Fabric API + Continuity Mod for Fabric for connected glass.
C. You can use Fabric + Sodium / Iris Shaders + Fabric API + Continuity Mod for Fabric + Indium for connected glass.

Option A :
Connected Glass with Clean and Clear Glass using OptiFine ~
1. Install OptiFine into your Minecraft Launcher.
2. Download Clean and Clear Glass in this CurseForge.
3. Open %appdata% > .minecraft
4. Move the Clean and Clear Glass .zip file into "resourcepacks" File Folder
5. Launch your Minecraft with OptiFine, activate the resource pack, and done.

Option B :
Connected Glass with Clean and Clear Glass using Fabric + Fabric API + CTMF ~
1. Install Fabric into your Minecraft Launcher.
2. Download Fabric API
3. Download Continuity for Fabric (CTMF)
4. Download Clean and Clear Glass in this CurseForge
5. Open %appdata% > .minecraft
6. Then, move all the .jar files that you downloaded before (Fabric API + Continuity) into "mods" File Folder
7. Move the C&CG for Fabric zip into "resourcepacks" File Folder
8. Launch your Minecraft with Fabric, activate the resource pack, and done.
