Misplaced criteria
The "Fragments of History" achievement is located in Outdoor Zones > Dragon Isles > The Waking Shores > Achievements is missing the "Dislodged Dragoneye" criteria, currently it is located in the Outdoor Zones > Dragon Isles > The Azure Span > Achievements.

Here is a detailed description of how each of the criteria work. Each criteria is tied to a zone, that then loots an item which can finally be turned in to grant achievement criteria reward

Wait, what version of ATT do you use? It was fixed yesterday in f5c8a7c and should be included in the latest 3.1.2 release.
This was on DF-3.1.2, with a hard refresh as I noticed this, but it was still present. I just relogged the game to confirm my above statement:
The updated coordinates from f5c8a7c seems to be present, but the criteria is still misplaced
