I was under the impression 15 was the highest level of keystone, but apparently not. Not sure if there's any cap at all, but the loot ilvl code is broken for levels past 15

There's no (known) cap as the system seems to be equal to D3's system :)
Maybe just use level 12 values if level >= 12?
