Error on Startup, locks step in guide

I get the following error when I startup or do a /reload.
I am using Maw Human Startert. The step that refuses to clear is:Discover Rolf's Fate. All steps after that will clear only on a /reload. I can not manually check a step off.
On a whim I deleted that step and the step before it from the guide at it got rid of the bug. There is nothing in them that I see that could cause issues.

In the case of the T step, I moved the QID from where it was after the Note before the Map to try to see if it was missing the QID after such a long note. No luck. I double checked Wowhead and that is the correct QID.
